Insurance programs currently accepted: VA Medicaid, WV Medicare, WV Medicaid, Unicare of WV. If your insurance plan is not listed please feel free to reach out we can begin the process of credentialing with your plan.
Physician Referral: Physician referrals are commonly required for participating insurance plans. You can download a physician referral form here, or have your doctor’s office fax your referral to (205) 660-6331.
Private Pay: For private pay clients, or those out of network who would like a superbill to submit to their insurance company for reimbursement, you can click below to set up your initial evaluation today.
